It is an offline application PDF form which is required for Online Apply under SSEPD Department's official webportal. In this scheme Government provides financial assistance of amount Rs. 20000/- to the bereaved household in the event of death of the bread-winner. It is clarified that any event of death (natural or otherwise) would make the family eligible for assistance.

• There should not have any income source in family
• Should have BPL Card
